POSITION SUMMARY:To provide social work services to clients admitted to the In-Patient Mental Health Program.
POSITION RESPONSIBILITIES
- To provide social work and discharge planning services to in-patient mental health clients and their families.
- To provide psycho-social assessments and plan effective intervention strategies for clients.
- To assess families and report client and families’ expectations regarding intervention and discharge plans to the inter-disciplinary team.
- To provide individual and family counselling and education on issues of adjustment to hospital, mental illness and community services.
- To know and function within standards of professional competence.
- To participate as a member of a multi-disciplinary health care team of Mental Health Services.
- To refer clients to the appropriate services and/or programs based on their assessed needs.
- To collect data for client tracking and performance indicator purposes.
- To consult regularly with members of the multi-disciplinary team regarding client status and service needs.
- To maintain effective public relations by making mental health resources known to the community and by utilizing resources of the community for the benefit of the client.
POSITION REQUIREMENTS:
- Baccalaureate in Social Work from a recognized university may be considered, M.S.W. preferred.
- Must be certified with the Ontario College of Certified Social Workers and Social Service Workers.
- Must be certified in CPI (Crisis Prevention Intervention).
- Minimum 3 years mental health experience in a hospital setting is required.
- Working knowledge of mental health legislation and related policies.
- Working knowledge of the signs and symptoms of mental illness, risk indicators, and the potential impact of mental illness on family members.
- Working knowledge of the relevant services within the mental health program and the community.
- Must possess the verbal ability to communicate with patients, physicians, program staff and other stakeholders in order to give and receive information or instructions and to write clear and accurate reports.
- Must have the ability to function effectively as a member of a multi-disciplinary health care team.
- Must be able to work flexible hours as determined by patient service needs.
- Bilingualism considered an asset.
- Employment history which demonstrates regular attendance at work.
- Bilingualism (French and English) would be considered an asset.
- All TADH staff members must follow the COVID-19 vaccination series in accordance with the most up-to-date vaccine guidance provided by the Ontario Ministry of Health.
- Starting salary of $44.67/hour; $87,106.50/year (As per Step 1 of the OPSEU collective agreement.)
- Option to enroll into Healthcare of Ontario Pension Plan.
