Position Summary:To provide pharmacy services according to the mission, objectives and policies of the Hospital. To uphold the Standards of Practice of the Ontario College of Pharmacists, the Guidelines of the Canadian Society of Hospital Pharmacists and the Standards for Accreditation Canada. Reports to the Pharmacy Manager.
Position Responsibilities
- Provide client-centered pharmaceutical care; actively evaluate client needs, with direct client participation, to ensure that drug therapy is achieving the desired therapeutic outcomes. Identify, prevent, and resolve drug related problems for individual clients.
- Participate in day to day activities of drug distribution such as order verification.
- Participate as an active member of the multidisciplinary, client-centered care team.
- Participate as Core Team member for designated program.
- Provide in-service and education for nurses, physicians, technicians, and other allied health professionals as required.
- Provide medication counselling.
- Monitor and report Adverse Drug Reactions, Medication Incidents and Near Misses.
- Be available as staff resource for drug information including dosing, administration, and compatibilities.
- Liaison with community agencies to foster seamless care.
- Consult with physicians to optimize drug therapy.
- Participate in drug utilization reviews.
- Commit to continuing education.
- Develop policy and procedure for drug related issues as required.
- Participate in Committees such as Drugs and Therapeutics, OHA District etc. as requested.
- Participate in Continuous Quality Improvement Initiatives as required.
- Support Pharmacy Technicians in their traditional and expanded scopes of practice.
- Supervise IV and Chemotherapy preparations.
Position Requirements:
- Current licensure with the Ontario College of Pharmacists.
- Previous hospital pharmacy experience or completion of a Hospital Residency Program.
- Experience in Chemotherapy is an asset.
- Experience with Meditech would be considered an asset.
- Available to provide after-hours on call service and weekend coverage on a rotational basis.
- Physically able to accomplish required tasks.
- Bilingualism (French and English) would be considered an asset.
- Competent in using a variety of drug information resources.
- Must have had at least one doses of the COVID vaccine.
- Starting salary of $48.21/hour; $94,009.50/year (As per Step 1 of the OPSEU collective agreement.)
- Option to enroll into Healthcare of Ontario Pension Plan.
